&lt;P align=center&gt;The 8.40 pm sleeping car express from Euston to Glasgow ST Enoch running under clear signals at about 55mph became completely derailed except for the last two vehicles.&lt;/P&gt; 
&lt;P align=center&gt;CAUSE&lt;/P&gt; 
&lt;P align=center&gt;Unsatisfactory nature of subsoil below the permanent way,which had been undergoing extensive maintenance work previous day.&lt;/P&gt; 
&lt;P align=center&gt;FORMATION&lt;/P&gt; 
&lt;P align=center&gt;Princess Coronation class pacific no 6225 Duchess of Gloucester with 12 bogie vehicles including 12 –wheel sleepers .&lt;/P&gt; 
&lt;P align=center&gt;DAMAGE:&lt;/P&gt; 
&lt;P align=center&gt;The engine turned over at 45 angle while the tender was turned on to it’s side The whole train except the last two vehicles was derailed.&lt;/P&gt; 
&lt;P align=center&gt;CASUALTIES &lt;/P&gt; 
&lt;P align=center&gt;Three passengers were killed.&lt;/P&gt; 
&lt;P align=center&gt;POSSABLE Safety measures.&lt;/P&gt; 
&lt;P align=center&gt;Temporary speed restrictions should be imposed after such work.&lt;/P&gt;</description>

&lt;P align=center&gt;&lt;FONT size=2&gt;Trailing crossover at Uddingston at Junction.&lt;/FONT&gt;&lt;/P&gt; 
&lt;P align=center&gt;&lt;FONT size=2&gt;The 1.30pm express passenger train from Euston to Glasgow became derailed whilst travelling at moderate speed.&lt;/FONT&gt;&lt;/P&gt; 
&lt;P align=center&gt;&lt;FONT size=2&gt;CAUSE:&lt;/FONT&gt;&lt;/P&gt; 
&lt;P align=center&gt;&lt;FONT size=2&gt;it had been necessary to isolate a defective brake cylinder at Carlisle ,but the carriage examiner had omitted to release the brake first .The wheels, which were at the front of the leading bogie on the fifth coach were therefore locked and during the onward journey a groove had been worn in the middle of the tyre creating a lip on the outside edge which damaged a wing rail as it passed through a trailing crossover causing the following coaches to become derailed..&lt;/FONT&gt;&lt;/P&gt; 
&lt;P align=center&gt;&lt;FONT size=2&gt;FORMATION&lt;/FONT&gt;&lt;/P&gt; 
&lt;P align=center&gt;&lt;FONT size=2&gt;Princess Coronation class pacific no 46235 City of Birmingham and 12 vehicles vacuum braked.&lt;/FONT&gt;&lt;/P&gt; 
&lt;P align=center&gt;&lt;FONT size=2&gt;DAMAGE &lt;/FONT&gt;&lt;/P&gt; 
&lt;P align=center&gt;&lt;FONT size=2&gt;six coaches were derailed the last two being severely damaged in sidelong collision with wagons on adjoining line.&lt;/FONT&gt;&lt;/P&gt; 
&lt;P align=center&gt;&lt;FONT size=2&gt;CASUALTIES:&lt;/FONT&gt;&lt;/P&gt; 
&lt;P align=center&gt;&lt;FONT size=2&gt;One passenger was killed four passengers and the guard were injured.&lt;/FONT&gt;&lt;/P&gt;</description>

&lt;P align=center&gt;location Down fast line just south of Stafford no 4 signal box.&lt;/P&gt; 
&lt;P align=center&gt;The 4.35 pm stopping passenger train from Coventry to Stafford was standing on the down fast line with engine opposite Stafford no 4 signal box when it was run into in the rear by the 3.45 pm express passenger train from Euston to Liverpool Lime street.&lt;/P&gt; 
&lt;P align=center&gt;cause&lt;/P&gt; 
&lt;P align=center&gt;signalman’s error,He accepted the Liverpool express before the stopping train had passed through the section.&lt;/P&gt; 
&lt;P align=center&gt;Formation&lt;/P&gt; 
&lt;P align=center&gt;The stopping train—2-6-4 tank engine no 2487 with four bogie vehicles lightly loaded with Passengers.&lt;/P&gt; 
&lt;P align=center&gt;The Liverpool express—Patriot class 4-6-0 no 5500 Patriot (pilot engine and Patriot class 4-6-0 no 5512 Benson(train)engine )with 13 bogie vehicles.&lt;/P&gt; 
&lt;P align=center&gt;Damage&lt;/P&gt; 
&lt;P align=center&gt;The near coach of the local train was telescoping between the second and third coaches .The express train was virtually undamaged .&lt;/P&gt; 
&lt;P align=center&gt;Casualties &lt;/P&gt; 
&lt;P align=center&gt;One Passenger died in Hospital and one was injured &lt;/P&gt; 
&lt;P align=center&gt;Signalling Absolute block with semaphore signals.&lt;/P&gt; 
&lt;P align=center&gt;Possible safety measures.&lt;/P&gt; 
&lt;P align=center&gt;Track circuiting and the associated controls would have presented this accident.&lt;/P&gt;</description>

            <description>&lt;P align=center&gt;3.7pm Saturday 21 july 1945&lt;BR&gt;LOCATION&lt;BR&gt;Trailing points with the refuge siding in the up main line.&lt;BR&gt;The 1pm express pasenger train from Glasgo to London passed the up distant signal at caution and the up outer and inner home signals at&amp;nbsp; danger and collided sidelong at about 60-65 mph with the fourthwagon of goods train which was being set back off the up main line into the refuge siding to clear the loine for the express.&lt;BR&gt;CAUSE;&lt;BR&gt;The Driver wrongly passed the signals at danger.&lt;BR&gt;CONDITIONS;&lt;BR&gt;Raining&lt;BR&gt;FORMATIONS;&lt;BR&gt;The express-Princess Coronation'class Pcific np 6231 Duchess&amp;nbsp; of Atboll,a bogiebrakevan 11 coaches and a bogie brakevan.&lt;BR&gt;The goods train -Standard Class 4f 0-6-0 no 4324 ,45 wagons and brakevan.&lt;BR&gt;DAMAGE&lt;BR&gt;The Pcific and the first vehicle wereoverturned ,and several coaches were derailed but remained upright and in line'&lt;BR&gt;CASUALTIES;&lt;BR&gt;The driver and the fireman of the Pacific were killed A total of 31 passengers were taken to hospital.&lt;BR&gt;SIGNALLING&lt;BR&gt;Absolute block with semaphore signals,A colour -light up distant signal had been planned prewar but had yet been installed.&lt;BR&gt;POSSABLE SAFETY MEASURES;&lt;BR&gt;ATC/AWS was the only reliable safeguard in case like this&lt;/P&gt;</description>

&lt;P align=center&gt;No 1&lt;BR&gt;Euston Station 26 April 1924&lt;BR&gt;Location Euston no 4 signal box up slow line&lt;BR&gt;The 7.15 am electric multiple-unit from Watford to Euston,running at 20-30 mph under clear signals,crashed into the rear of the 5.30am cup final excursion from Coventry to Euston,which was standing at Euston No 4 signal box up slow line home signal ,Although all the coaches of the excursion train were gas -lit ,there was no fire.&lt;BR&gt;CAUSE:&lt;BR&gt;Signalman's error.&lt;BR&gt;The signal man at Euston No 4 signal box had just dealt with an up Scotch express on the up fast line,but he sent the Train out of Section's signal for that train to Camden No 1 signal box on the up slow line block instrument in error.&lt;BR&gt;He was then offered ,and accepted the 7.15 am EMU from Watford,resulting in the collision.&lt;BR&gt;FORMATION :&lt;BR&gt;The Watford train was a six -car EMU.&lt;BR&gt;casualties:&lt;BR&gt;five passengers were killed and 68 were injured.&lt;BR&gt;SIGNALLING&lt;BR&gt;Absolute block .There was no track circuits at Euston No 4 up home signals .The signal box had an electrically operated minature lever frame.&lt;BR&gt;POSSABLE SAFETY MEASURES&lt;BR&gt;A track circuit at the up slow line home signal with appropriate signalling controls,would have prevented the accident.&lt;BR&gt;&lt;/P&gt;</description>
